Jessica Collins

I attended Purdue University for my Bachelor's Degree. Since 2015 I have worked as a Case Manager for individuals within the Family Support Waiver and CIH waiver programs. I have 2 small dogs: a French bulldog & yorkie mix, and a Pigeon.   For 2 ½ years I worked as a Care Coordinator working with at-risk youth & families coordinating services, completing CANS assessments to identify the most immediate needs, referred and coordinated services to build a team that would ensure the safety, stability, and permanency for every family/child. As a team, we worked together to develop a family driven care plan to ensure maximum success in the community. I specialized in medically fragile cases, especially diabetes, and worked with medical professionals in developing a successful plan of care. For 3 years prior, I acted as a Court Appointed Special Advocate (CASA) reporting to the courts the best interest for my clients.
