1. My Case Manager responds to me within a 24-hour period.
2. I have had a Virtual and/or In-Person visit from my Case Manager at least every 90 days.
3. My Case Manager encourages me to state my opinion and advocate for myself.
4. I am given choice of waiver services and providers that are available to me.
5. I am comfortable talking to my Case Manager about my needs.
